Collaboratives
SGPN is offering general practices the opportunity to be part of the successful Australian Primary Care Collaborative program at a local level.

The national program has enabled participating general practices to achieve significant improvements and measurable results in the following areas:
  • improved patient care through better management of diabetes and coronary heart disease
  • improved use of medical and business information systems
  • improved patient access to primary care
Twenty local practices are currently participating in the program and we to continue recruitment of practices throughout 2010. Collaborative training days or ‘learning workshops’ are held quarterly in Canberra.

Contact our Practice Support team to hear more about the local collaboratives.

The Australian Primary Care Collaborative website has information about the program and the collaborative methodology.
